Arrrggghhh! I didn’t look carefully enough and missed this being an anamorphic state! I’d go for Xylaria cubensis (Xylocoremium state). You could, quite correctly, use just Xylaria cubensis, but I like the idea having a separate pigeonhole for those observations that show only the anamorphic form. I did something like this for the ACT where I added the ‘species’ Septobasidium clelandii (Harpographium state). In the field guide the observations of the anamorphic and non-anamorphic states would be right alongside each other.

In your notes you say Xylocoremium flabelliforme. I don't know the species (nor the genus) but having looked at some photos, I can well see why you think so. Therefore, it's a great shame to put this under the genus Xylaria. Why not add Xylocoremium flabelliforme to the list of fungi and confirm it?
